Care Coordinator II

The Care Coordinator II is responsible for providing individualized community support services to clients referred from the Office of Aging and Adult Services. The goal is to assist individuals and/or households to obtain and maintain housing successfully within the community.

Essential Duties and Responsibilities:

  • Develop, implement, monitor, evaluate and modify a person-centered housing support plan to ensure consumers are receiving services as described in contract.
  • Ensure accurate recordkeeping including but is not limited to intake; service planning; service delivery including advocating for clients with property owners/managers, works closely with other community service providers working with each tenant, and providing resource information; and case closure.
  • Prepares and completes accurate, timely reports and documentation for billing of services.
  • Ensuring compliance with the weekly performance standards, billing 100 units/25 hours per week for 52 weeks, inputting into service point system within 24 hours as required by Volunteers of America Southeast Louisiana and regulatory agencies/funding sources.
  • Ensures eligibility of services prior to performing home visits and off-site case management meetings, providing transportation to consumers as appropriate.
  • Assist clients in accessing and maintaining entitlements/benefits and meeting obligations of tenancy.
  • Provide crisis response and intervention.
  • Must maintain assigned up-to-date client files and program records.
  • Participate in training required by accreditation standards, plus performance and quality improvement efforts by adherence to Performance, CARF, and Quality Standards.
  • Performs related duties as assigned.

Additional Duties and Responsibilities (Non-essential):

  • Adhere to the designated dress code as stipulated for the specific role.

Supervisory Duties:

  • Supervises and takes lead on an assigned caseload.
  • Communicates performance expectations to peer support by providing specific and measurable ongoing feedback.
  • Identify strengths to build on the peer support individual performance for the future.
  • Supervise services being provided to members/consumers by a Peer Support Specialist.

Key Contacts:

Maintains critical relationships with the following:

  • Serves as primary representative of the Agency with respect to third party vendors providing training-related products and services.
  • Utilizes the Relias Management System to maintain compliance of all required training courses.

Knowledge, Skills and Abilities:

This position requires knowledge, skills and abilities equivalent to:

  • Completed education at the bachelor's degree level in social service field and completed a year post experience working with individuals who are of the eligible population in a community setting.
  • Have the desire to work with a diverse population, demonstrate respect for individuals and demonstrate ability to work in a team environment.
  • Demonstrated willingness to be responsive to the needs of the target population.
  • Excellent verbal and written communication skills and the ability to function as part of the team.

Travel to other locations as appropriate in the performance of duties and responsibilities.

Must have:

  • Current driver's license issued by state of residence.
  • Current vehicle Inspection decal.
  • Current vehicle insurance.
  • Current vehicle registration in state of residence.
  • Satisfactory Motor Vehicle Report.
  • Must be able to respond to consumer's needs outside of normal working hours.
  • Must be able to obtain certification in Cardio Pulmonary Resuscitation and First Aid.
  • Must be able to pass a criminal history investigation.

  • The employee is occasionally required to lift up to 25 pounds when picking up food boxes, preparing/transferring training materials, and equipment.
  • Visual acuity is necessary to operate computers, audiovisual and other digital equipment.
